NurseRegistry Semi-Annual Survey (July 2024) Official Entry Rules
Terms and Conditions
1. Eligibility:
Participants must be legal residents of the United States and at least 18 years old as of the date of entry.
Participants must be current or past clients (private duty or staffing) or nurses associated with NurseRegistry.
Employees of NurseRegistry and their immediate families are not eligible to participate.
The survey giveaway is subject to all applicable federal, state, and local laws and regulations. Void where prohibited.
2. How to Enter:
No purchase is necessary to enter or win. To enter, complete the NurseRegistry customer or nurse satisfaction survey in its entirety, via the URL emailed to you, between July 16th, 2024 and July 31st, 2024. Limit one entry per person.
All surveys must be submitted by 11:59 PM PT on the end date to be eligible for the drawing.
3. Drawing Date:
One (1) winner will be selected in a random drawing on or about August 7th, 2024 from all eligible entries received. Odds of winning depend on the number of eligible entries received.
4. Prize:
Two (2) winners will receive a $300 debit card. One (1) winner will be selected from the client survey and one (1) winner will be selected from the nurse survey.
Prize is non-transferable. If the winner is unable to accept a corporate gift, NurseRegistry will make a charitable contribution in their honor to a recognized charity of their choice. No cash or other prize substitution shall be permitted except at NurseRegistry’s discretion.
Arrangements for the prize or charitable donation will be made once the winner has been selected and notified.
5. Winner Selection and Notification:
The winner will be selected in a random drawing from all eligible entries received.
The drawing will be conducted within seven (7) days after the end of the entry period.
The potential winner will be notified by email or phone within 7 days of the drawing date. If a potential winner cannot be reached after reasonable efforts have been made (as determined by sponsor), an alternate winner may be selected.
6. General Conditions:
NurseRegistry reserves the right to cancel, suspend, and/or modify the drawing if any fraud, technical failures, or other factors impair the integrity or proper functioning of the drawing, as determined by NurseRegistry in its sole discretion.
NurseRegistry reserves the right to disqualify any individual it finds to be tampering with the entry process or the operation of the drawing or to be acting in violation of these Official Entry Rules or any other promotion, or in an unsportsmanlike or disruptive manner.
All decisions by NurseRegistry regarding the drawing are final and binding.
By participating, entrants agree to abide by these Official Rules and all decisions of the sponsor, which shall be final and binding in all matters relating to this drawing. Void where prohibited by law.
7. Privacy:
Information collected from participants is subject to NurseRegistry’s privacy policy. By entering the drawing, participants agree to the terms of the privacy policy. To read the Privacy Policy, click here.
8. Release and Limitations of Liability:
By participating in the drawing, entrants agree to release and hold harmless NurseRegistry, its affiliates, employees, and agents from any and all liability, injury, loss, litigation, claim, or damage of any kind arising from or in connection with the drawing or receipt or use of any prize.
9. Disputes:
All disputes, claims, and causes of action arising out of or connected with the drawing or any prize awarded shall be resolved individually, without resort to any form of class action, and exclusively by the appropriate court located in California.
All issues and questions concerning the construction, validity, interpretation, and enforceability of these Official Entry Rules, entrants’ rights and obligations, or the rights and obligations of NurseRegistry in connection with the drawing, shall be governed by, and construed in accordance with, the laws of the State of California, without giving effect to any choice of law or conflict of law rules.

11. Agreement to Rules:
By participating, the Contestant (“You”) agrees to be fully unconditionally bound by these Rules, and You represent and warrant that You meet the eligibility requirements. In addition, You agree to accept the decisions of NurseRegistry as final and binding as it relates to the content of this drawing.
