What to Expect Next
Sign the Service Agreement
You will be emailed NurseRegistry’s “Client Service Agreement” or “Client Facility Agreement.”
The Client Service Agreement helps outline our scope of services, which includes acting as your designated agent for:
- Identification and placement of independent nursing contractors
- Scheduling and/or logistical assistance
- Billing and payroll for the independent nurse
The Client Facility Agreement outlines NurseRegistry’s specific responsibilities.
Our Team Begins Looking for a Nurse
Based on your specific needs, we will handpick nurses that match your requirements. We consider skills, experience, personality, age, and gender.
*Note – NurseRegistry can NOT begin this process until you have signed the Client Service Agreement or Facility Service Agreement.
Wound Care/IV Requirements
In the case of wound care or IV therapy, NurseRegistry requires the following in addition to a signed contract:
- Written doctors orders
- Discharged paperwork
- All necessary supplies to complete care
Questions to Prepare for (Hiring a Private Nurse)
If you are in the process of hiring a private duty nurse from NurseRegistry, here is a list of questions you may be asked:
- Are you within the state of California?
- Which county and city are you in?
- Is this a singular visited or ongoing service?
- How many days per week do you require a nurse?
- How many hours per day?

- What is your preferred time slot?
- What specific tasks should the nurse expect to complete?
- Do you have a preference regarding the nurse’s gender?
- Do you have a vaccination preference?
- Does the nurse need to wear a mask while inside the residence?

Questions to Prepare for (Nurse Staffing for a Facility)
If you are in the process of hiring a private duty nurse from NurseRegistry, here is a list of questions you may be asked:
- Is the facility in the state of California?
- What county of California are they in?
- What type of facility is this?
- Do you need to hire an LVN or RN?
- When do you need the care to start?
- Will there be an ongoing need for nurse staffing?
- Do they need supplemental coverage or full-time coverage?
- How many days per week do you need a nurse at this moment?
- What is the typical shift length? Does that apply to morning, night, and over-night shifts?
- Are there any COVID-related requirements that the nurses should know about?
- What is the nurse-to-patient ratio?
- Is there a particular dress code the nurse must adhere to?
- What primary duties will the nurse be expected to complete?
What to Note in the Contract
What are the Nursing "Tiers?"
Nurse Tiers are priced differently due to how the private independent nurse has valued themselves. The tier is not reflective of a nurse’s experience or skills.
Cancellation Policy
Clients are required to cancel shifts/visits 12 hours or more before the shift/visit is scheduled to begin. If the client fails to do this, they will be billed a minimum of 2 hours or half the shift (max. of 4 hours), depending on the length of the agreed-upon shift.
Scheduling Minimums
Yes. There is a two-hour minimum. However, scheduling can be flexible depending on your needs.
Complete Confidentiality
NurseRegistry shall protect the confidentiality of all records and other materials containing personal identifying information.
The Contract is NON-Binding
It is important to note that your contract with NurseRegistry is nonbinding. There are no fees and you may leave the working relationship at any time.
Time Records
The Independent Contractor will maintain records of hours worked. It is the client’s responsibility to review and approve the time submitted by the Independent Contractors. Failure to do so will be considered an approval of the time submitted.
Holiday Rates
Should a Client request work on holidays agreed upon by NurseRegistry, the Client will be billed time-and-a-half rate.
This includes, but is not limited to: New Year’s Day, Easter, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Christmas Eve (after 4 pm), Christmas Day, and New Year’s Eve (after 4 pm).
Hourly Rate Changes
The hourly rate may be changed upon 30 days written notice to the Client or legal representative.
Direct Hire
In the event a Client wishes to employ any Contractor(s) directly, the Client will be subject to a fee of $15,000.00 or 20% of the Contractor’s annual salary (whichever is greater). This is due within 10 business days of direct employment.
Notice of Assignments
NurseRegistry shall give the Facility at least two hours notice regarding assignments that NurseRegistry cannot provide Contractors for.
Cannot Guarantee Coverage
We will do everything in our power to provide highly-qualified and skilled nursing to cover your required shifts. However, depending on availability and the location of your request, we can never absolutely guarantee coverage.
General FAQ
Are services available outside of the home?
Yes, our nurses can provide skilled care outside of the home. Private duty nursing is available in nursing homes, skilled nursing facilities, assisted living facilities, hospitals, retirement communities, or directly in the home.
Is NurseRegistry a skilled nursing facility (SNF)?
No, NurseRegistry is not a skilled nursing facility. While our nurses do provide skilled care, we do not operate a residential facility. We cannot provide rooms for your loved one; our nurses go directly to where you most need care—whether that be in the comfort of your own home or supplemental care in a facility.
What Records does NurseRegistry Keep on Nurses?
NurseRegistry maintains records, such as:
- Complete application, which includes education, training, skills, specializes, and preferences
- Documentation of education and training
- Skills inventory checklist
- TB Test and evidence of satisfactory health status
- Current CPR
- Copy of current license, registration or certification
- Criminal background check results
- Proof of malpractice and general liability insurance
Do you screen your nurses?
Yes. All of our nurses are licensed and insured. Nursing licenses are validated through a current search of state records. Nurses must pass a background check (local and national criminal records) and nationwide sex offender registry check. Is there a minimum in terms of time?
Yes, there is a minimum of two hours per visit; however, scheduling can be flexible to your needs.
Are services covered under insurance?
At this time, we are private pay only. However, we do work with our clients to assist in reimbursement from both primary health and long term care insurance. Please contact your individual insurance carrier to confirm eligibility.
What if I need to reach someone after hours or on weekends?
We have dedicated 24/7 support staff who are available nights and weekends. If you reach our voicemail or answering service, please leave your information and our on-call coordination team member will reach out to you as quickly as possible.
What is the difference between a caregiver and a nurse?
The difference between caregivers and nurses is that nurses provide medical care, while caregivers can not. Caregivers are professionals that assist with activities of daily living (ADLs). Nurses are licensed medical professionals who provide skilled medical care in addition to support with ADLs. Often, a licensed nurse lowers anxiety and stress for clients and their family members.
What is the difference between an RN and an LVN?
A Registered Nurse and Licensed Vocational Nurse differ in scope of practice. Both are able to provide skilled medical care; however, only an RN can administer intravenous (IV) medications.
Are NurseRegistry services only available for seniors?
No, NurseRegistry services are available for people of all ages, including seniors, adults, adolescents, children and infants.
Is someone able to give me advice over the phone?
We are not a nurse hotline, and we cannot answer medical questions over the phone.
Can NurseRegistry help me in an emergency?
If you are in a true state of emergency, please dial 911 or visit your closest Emergency Department. In non-emergency situations, we will always do our best to get a nurse to you as quickly as possible.
